You have to start with the box. How deep and wide do you want it? Do you use plywood or hard wood? Do you use screws or nails? I chose some 1x boards and used mainly screws. Second time in, I created some compartments for securing stuff on the sides.
You need to secure the box to the ladder, and I chose to use some bolts and wing nuts. To make it more secure, I permanently affixed a 2x10 board to the top of the ladder and secure the box to it. Easier to twist the wing nuts outside rather than under the ladder top, as you can see below:

The last thing to consider is comfort. Do you want your precious ones sitting on wood? Not me! I saw carpet and felt and all kinds of materials, but I chose a piece of foam that I enclosed in some faux leather. I made it for the bottom and back. The back is really unnecessary.
I wish I had made the overall box a little deeper. As my kids got older, their legs hung uncomfortably at the middle of their thighs. I supported it with another piece of wood for a few more inches but ultimately I re-built the top.
